Understand systems before they fail

Problems rarely arise suddenly. The signals are usually visible long before they break out: small deviations, recurring friction, informal workarounds, or incidents that get treated as one-offs. The real challenge is recognising these signals early and interpreting their meaning correctly.

In complex organisations, simple cause-and-effect explanations often fall short. Safety doesn’t emerge from processes or controls alone. It emerges from the interplay of people, structures, decisions and organisational conditions.

Treating only symptoms rarely improves the system. That’s why we need analyses that make patterns visible: not analyses that only check formal compliance.

Through analyses, assessments and reviews, I help organisations identify risks early, surface weaknesses and pinpoint effective areas for action. The aim isn’t to create additional bureaucracy, but to enable better decisions based on a realistic understanding of how the system actually works.

  • Risk analyses and systemic risk assessments
  • Culture and safety assessments
  • Lessons learned and learning-from-events reviews
  • Reviews after incidents or critical events
  • Analysis of interfaces, dependencies and governance structures
  • Support with organisational status assessments

The goal isn’t more control, but more clarity, before problems turn into crises.
